Explore thoroughly, delay using the books until Master level using them all at once , make a point of reaching key areas like Ysa, Rathir and Adessa before you run out of places to put skill increases at level-up without blowing off trainers and you will be fine. There are 2 low level trainers for each skill, one medium and one or two with Dead Kell in 3 skills at master level, plus one book per skill.
As far as finishing, make a note of master skill increases available in Dead Kel and make certain that after using all trainers and books in the main game that you only need 2 or 3 of the 3 provided by that DLC in order to max all. If you want to push it, like using some books earlier rather than later then it might serve you well to create a simple chart on a piece of paper to keep track of trainers visited, books found and books used. As far as maxing lorestones, if exploring everywhere does not do it then be patient.
When you reach the top level of "Detect Hidden" that will make them appear on the map. If you are also maxing skills then you will not come across the top level in that until later, but at that time, once it is eventually maxed, you can use the ability to rapidly dust off any lorestones you missed along the way.

You need to find them. You will if you do everything in the main game. You can use the books at any time but I find that I need those non-trainer spots at the basic and advance skill levels for level-ups and so I save them.
Be careful if you use the book for a skill before master level, that you do not do so in a way that causes you to lose the benefit of one othe trainers I just put the books I find in the stash until I have them all and finish advance level for each skill.
Doing that means the ones I have in stash are the record and I do not need a paper record as well. The main game has one trainer in each skill at the master level.
